THE PROJECT

Connect prescriptions, devices and the session itself.

Designed around clinic-based sessions and home use, the system connects clinician workflows, PICO headsets, Bluetooth HRV devices and immersive content. Development spans the Windows clinician client, Unity/PICO applications, backend interfaces and device communication: clinicians configure tasks, the headset runs them, and progress and physiological readings return as linked session records.

01 / HIS CLINICIAN WORKSPACE

Prescribe the session. Follow its progress.

The clinician workflow brings patients, prescriptions and devices together, combining activity planning, headset allocation, screen viewing and session records.

Prescriptions and scheduling

Patient-linked prescriptions
Link the patient record, attending clinician, training plan and assigned headset so the plan and its execution can be reviewed together.
Activities and execution parameters
Sequence breathing, nature relaxation and exposure activities. Configure duration, repetitions and level, with an overview of the total session time.
Scheduling and task delivery
Set planned sessions, frequency, start date and location, then deliver the selected environment and execution parameters to the assigned headset.
PRESCRIPTION / Configure activities, parameters, scheduled sessions and the target headset.

Device overview, screen viewing and HRV

Device cards and screen viewing
Find devices by online, active, idle, offline or loan status. Cards show battery, patient association, current content and progress, with a direct action to view the headset screen.
Bluetooth HRV integration
Scan, select and connect the sensor, read its data, and display HRV as RMSSD in milliseconds alongside connection status. Readings are associated with the current session.
Two-way communication and records
WebSocket and JSON messages carry launch commands and training parameters to the headset, and return device status, current environment, progress and session records. Loaned devices retain their own loan, return and last-sync information.
DEVICE OVERVIEW / Device status, headset screen viewing and HRV (RMSSD, ms) in one workspace.

The deployed clinician application is a Windows client on the customer’s local network. The two clinician visuals above are reconstructed views of the project workflow.

02 / VR MANAGEMENT HUB

A clear starting point for every session.

The hub receives clinician-assigned activities and provides an entry point for patients to access available content. The management interface and lounge connect plan review, device connections, task launch and the return after a session.

Home therapy management

Patient workspace
Bring today’s plan, activity entry points, accumulated training time, the calendar and past sessions into one patient workspace.
Device and data connections
Show the PICO identifier, battery and HIS connection alongside sensor status and RMSSD readings. Organize records by patient, device and session.
HOME MANAGEMENT / Daily activities, the session calendar and device connection details.

Lounge and session orchestration

Clinic and home entry points
In clinic mode, wait for the clinician’s assigned task; in home mode, enter available activities through the panel. The lounge provides a familiar place for instructions and the start and end of each session.
Scene orchestration and recording
Receive the environment, level, duration and repetitions, then handle launch, completion and return to the hub. Store start and end times, duration and process data locally as JSON, and upload when the connection is available.
MAIN LOUNGE / Connecting the waiting area, management panel and training environments.

03 / INTERACTIVE VR ENVIRONMENTS

From handling objects to facing an audience.

Graded exposure tasks become interactive sequences through environmental intensity, object manipulation, character responses and guidance. Clinicians configure level, duration and repetitions; the VR application runs the task and links its progress with HRV readings from the same session.

Contamination exposure environments

Lower intensity · living room
Observe and teleport around a tidy but unsanitized living room, touch furniture and pick up a book or cup to complete initial contact tasks.
Intermediate intensity · kitchen
Food residue, dirty worktops, unwashed dishes and standing water increase the visible contamination. Picking up rotten fruit adds a more direct contact task.
Higher intensity · bathroom
Stained fixtures, scattered waste and insect sounds create a more intense setting. The participant teleports next to the toilet, observes it up close and picks up the toilet brush.
Interaction feedback and response prevention
Virtual hands, object grabbing and controller vibration provide feedback. Timed tasks that delay washing, text prompts and audio guidance structure the exposure and response prevention (ERP) workflow.
CONTAMINATION EXPOSURE / Picking up fruit and interacting with objects in the intermediate kitchen environment.

Social anxiety exposure environments

Lower pressure · campus encounters
Students arrive and wave, nod or smile. Selecting an NPC makes the character turn toward the participant and respond through gaze, expression, movement and a greeting.
Intermediate pressure · public speaking
Pick up the microphone and address an audience. Attentive listening, side conversations, noise and desk tapping create different degrees of distraction and audience feedback.
Higher pressure · examination room
The sequence moves from students arriving and the teacher explaining the rules to paper distribution, the bell and the task itself. The participant handles the pen and paper while the teacher patrols and other students remain present.
Dynamic NPC responses and guidance
Scripted rules organize character actions and responses, including audience reactions linked to physiological monitoring parameters. Text, voice and breathing guidance support the sequence, while screen viewing and HRV records provide the clinician with session context.
SOCIAL EXPOSURE / An examination setting combining the teacher, peers, ambient sound and task progression.

DEVELOPMENT & INTEGRATION

Immersive content connected to a working system.

The project connects prescription planning, VR task execution, device communication and session reporting. Delivery brings together management software, immersive interaction and hardware data. HRV is recorded as RMSSD in milliseconds and linked with session progress for review and record management.
